Diabetes checks planner

Welcome to my Diabetes Health Checks Planner

You should be getting 9 diabetes health checks on the NHS every year. This is sometimes called your annual review and is done by your GP or diabetes hospital team.

The results of these checks help you and your healthcare team understand how type 2 diabetes is affecting your body. You can then agree ways to manage your condition.

Diabetes can affect any part of your body. But if you have type 2, problems with your eyes, kidneys, and feet are most common, along with heart attacks and strokes. Health problems can start long before any symptoms. So, even if you feel well, it’s still important to get all your checks and find any potential problems early.

Use this planner to find out about each of the checks. You can record the ones you’ve had and the ones you need to ask about. When you reach the end, you'll have a record that you can take with you to your next appointment.
